Hours: 8:00 a.m. to 5:00 p.m. Monday through Friday

New patients are requested to arrive 10 minutes prior to appointment time to fill out new patient paperwork (please feel free to fill this out in advance; documents can be downloaded here).

24 hour cancellation notice is required. There is a $75 fee applied for a missed appointment if cancellation notice is not given.

Our staff works very hard to keep our schedules running on time, and to serve our patients on time. Your prompt appointment arrival is appreciated and will assist us in keeping other appointments on schedule.

Please bring all documentation of insurance (cards, policy numbers, phone numbers, etc.) so we can bill insurance for you.
